The Wyoming Department of Health has immunization requirements for those who attend child care, pre-school, and school. The state’s immunization requirements follow the current schedule published by the Centers for Disease Control and Prevention (CDC) and endorsed by the American Academy of Pediatrics (AAP) and American Academy of Family Physicians (AAFP).

Children enrolling in child care facilities, pre-school, pre-Kindergarten

Infants entering child care facilities must be up to date at the time of enrollment and are required to provide an updated certificate after completing all of the required vaccines due no later than 18 months of age.

  • Hepatitis B (HBV)

  • Diphtheria-Tetanus-Pertussis (DTaP, or DT if appropriate)

  • Pneumococcal conjugate vaccine (PCV) – age younger than 5 years only

  • Measles, Mumps, Rubella – 2 doses of each, normally given together as MMR

  • Varicella – 2 doses or credible history of disease

Children enrolling in Kindergarten
  • Hepatitis B (HBV)

  • Diphtheria-Tetanus-Pertussis (DTaP, or DT if appropriate)

The form certifies that a child has a condition that prevents him/her from getting vaccinated. The medical reason for the exemption does not need to be provided.

Religious Exemptions
Religious

This exemption requires a signed statement by the parent/guardian that vaccination conflicts with their religious tenets or practices. A written objection from the parent or guardian of a student based on religious tenets and practices shall be submitted in person by the parent or guardian to the County Health Department for issuance of a Certificate of Religious Exemption from the required immunizations or testing.

The Certificate of Religious Exemption and the Certificate of Medical Exemption will be on forms approved by the Wyoming Department of Public Health and will be accepted in lieu of the Certificate of Immunization.

Medical

To claim an exclusion for medical reasons, the child or student must present an exemption statement to the school or child-care facility, dated and signed by a physician (M.D. or D.O.), properly licensed and in good standing in any state in the United States who has examined the child or student. The statement must state that, in the physician’s opinion, the vaccine required is medically contraindicated or poses a significant risk to the health and well-being of the child or student or any member of the child’s or student’s household. Unless it is written in the statement that a lifelong condition exists, the exemption statement is valid for only one year from the date signed by the physician.

Religious

To claim an exclusion for reasons of conscience, including a religious belief, the child’s parent, legal guardian, or a student 18 years of age or older must present to the school or child-care facility a completed, signed and notarized affidavit on a form provided by the department stating that the child’s parent, legal guardian, or the student declines vaccinations for reasons of conscience, including because of the person’s religious beliefs.

Children who are new enrollees in a MS school in grades other than Kindergarten

Immunization Exemptions

Mississippi law allows exceptions for medical reasons.

Medical

Physicians (MD or DO) or Public Health Nurses are authorized to indicate specific vaccines medically exempted (because of risk of harm) on the certificate. Other vaccines remain required. The medical reason for the exemption does not need to be provided.

  • Medical exemption requests completed and signed by a tertiary care physician outside of the state will be reviewed on a case by case basis.
  • The same pediatrician, family physician, internist, or tertiary care physician must indicate on Form 139 the medical condition of the child seeking exemption and indicate the exemption status for each of the listed vaccines.
  • Form 139 must be submitted to the Mississippi State Department of Health Epidemiologist or Deputy State Epidemiologist.
  • The State Epidemiologist or Deputy State Epidemiologist will mail a copy of the Certificate of Medical Exemption (Form 122) to the parent and the pediatrician, family practitioner, internist, or tertiary care physician who submitted the Medical Exemption Request. The original Certificate of Medical Exemption (Form 122) will be housed at MSDH.

The Arkansas Department of Health has immunization requirements for those who attend child care, pre-school, and school. The state’s immunization requirements follow the current schedule published by the Centers for Disease Control and Prevention (CDC) and endorsed by the American Academy of Pediatrics (AAP) and American Academy of Family Physicians (AAFP).  

Children enrolling in child care facilities, pre-school, pre-Kindergarten

Infants entering child care facilities must be up to date at the time of enrollment and are required to provide an updated certificate after completing all of the required vaccines due no later than 18 months of age.

  • Hepatitis B (HBV)

  • Diphtheria-Tetanus-Pertussis (DTaP, or DT if appropriate)

  • Poliomyelitis (IPV or OPV)

  • Haemophilus influenzae type B (Hib) – age younger than 5 years only

  • Pneumococcal conjugate vaccine (PCV) – age younger than 5 years only

  • Measles, Mumps, Rubella – 1 dose of each, normally given together as MMR

  • Varicella – 1 dose or credible history of disease

  • Hepatitis A – 1 dose, required by 18 months of age or older

Children enrolling in Kindergarten
  • Hepatitis B (HBV)

  • Diphtheria-Tetanus-Pertussis (DTaP, or DT if appropriate)

  • Poliomyelitis (IPV or OPV) – final dose on or after the 4th birthday

  • In addition to the vaccines requirements listed below, one dose of Meningococcal vaccine is required upon entering the 7th grade

Children who are new enrollees in a AR school in grades other than Kindergarten
  • Hepatitis B (HBV)

  • Diphtheria-Tetanus-Pertussis (DTaP, or DT if appropriate)

  • Poliomyelitis (IPV or OPV – final dose on or after the 4th birthday

  • Measles, Mumps, Rubella – 2 doses of each, normally given together as MMR

  • Varicella – 2 doses or credible history of disease

  • New students entering grades other than 7th grade are not required to have Tdap

The medical reason for the exemption does not need to be provided. The form certifies that a child has a condition that prevents him/her from getting vaccinated. A physician statement outlining medical reasons for an exemption of a specific immunization is to be submitted to the Medical Director of the Communicable Disease/Immunization Section with the exemption form.

Religious

Religious exemptions can be granted for children “on the grounds that such immunization conflicts with the religious tenets and practices of a recognized church or religious denomination of which said parent or guardian is an adherent or member” in accordance with Arkansas Statute.

The Alabama Department of Health has immunization requirements for those who attend child care, pre-school, and school. The state’s immunization requirements follow the current schedule published by the Centers for Disease Control and Prevention (CDC) and endorsed by the American Academy of Pediatrics (AAP) and American Academy of Family Physicians (AAFP).

Alabama law requires that students must present a current blue Alabama Certificate of Immunization prior to enrolling in school.

Prior to entering Kindergarten students should have:
  • 2 doses of measles-containing vaccine

  • 1 dose of Varicella vaccine (unless the student has had documentation of Varicella disease)

  • Any other vaccines listed on the immunization schedule.

Prior to entering 6th grade students must have:
  • Aa Tdap vaccine for tetanus and pertussis (whooping cough).

A written objection from the parent or guardian of a student based on religious tenets and practices shall be submitted in person by the parent or guardian to the County Health Department for issuance of a Certificate of Religious Exemption from the required immunizations or testing.

The Certificate of Religious Exemption and the Certificate of Medical Exemption will be on forms approved by the Alabama Department of Public Health and will be accepted in lieu of the Certificate of Immunization.
